Phase 1. Preparation of donor area
At this phase of hair transplantation, first photograph is taken and then the area where the hair will be transplanted is determined and drawings are made on it. Following the shaving of hair, local anesthesia is applied to donor area. Meanwhile, the patient stretches on the operation chair, can watch TV, and most of them sleeps during operation.
The donor area from where the hairs will be taken should be prepared with extreme care considering the possible hairlessness problems in the future. It is started to take the hair bulbs, and thin scar in this area is closed by stitching. The width of scar is of a width as a line drawn by pencil. In order to minimize the thickness of scar, a new closure method called trichophitic is used. By this method, the lower boundary of the wound is made superficially inclined and thus the hairs grow right under the upper boundary of wound when it is closed with stitches. In this manner, the hairs grow on the wound and thus a thinner scar is obtained. This technique is being used for a while in plastic surgery, in eyebrow lifting operations and in minimizing the scar. The taking of donor hairs takes about 3 hours and a short break is given.
Phase 2. Preparation of areas where hair will be transplanted
Following the break, bulb hair transplanting phase starts. There many types for the areas where transplantation will be performed. Model of hair line, age, properties of hair, form of head, properties of face, requested density and flexibility of transplantation process are significant in determining the type and distribution of transplantation areas.
We are using various instruments in order to constitute the areas where the hairs will be transplanted (void ended needle, microtome knives which can generate cuts of 0.9-1-2 and 3 mm, staplers which can sometimes open holes of 1.3-1.5 mm and rarely of 2.00 mm). At this phase, the positioning of hair bulbs regarding frequency and density and their inclination of growing are significant issues. Right at this point, in order to prevent growing of hair similar to grass man, it gains importance for it to be performed by a doctor who performs well the calculation of properties of hair and of the cuts to be opened, and who has high level of specialization and aesthetic concern.
Phase 3: Preparation of hair bulbs to be transplanted
Technically the preparation of hair bulbs is not hard. The hairy tissue should be kept cold and humid within a special solution.
Phase 4: Placement of hair bulbs
The placement of hair bulbs is performed by using specially designed thin forceps. During the placement of hair bulbs, the technician team and doctors consisting of 2 or 3 individuals being by the patient place the hair bulbs to the area alternately and in order. The placement of hair bulbs requires experience and soft touch. Doctors and assistant technicians should have that handicraft, and it should be worked without haste and by resting in order to obtain the best result.
